Benefits

Employees working 20 or more hours per week are eligible for Health insurance, Dental insurance, Group and Supplemental Life Insurance benefits beginning the first of the month following a 30 day waiting period.

HEALTH INSURANCE – POS Plan
Three Plan Designs – but all cover the same services

  • $20/$35 co-pay for office visits/specialist visits (waived for annual preventive care visits)
  • $10/$25/$40 Rx co-pay (waived for certain preventive maintenance Rx)
  • Out-of-network benefits

Aetna CHOICE I - In-network services covered at 100% after co-pays. In-Network Hospital Co-Pay out-of-pocket maximums: $1,500 individual/$3,000 family.

Aetna CHOICE II - In-network services covered at 80%. In-Network, out-of-pocket maximums: $1,500 individual / $3,000 family.

Aetna CHOICE III - In-network services covered at 80% after individual $500 / $1,500 family deductible is met.  In-Network, out-of-pocket maximums: $1,500 individual / $3,000 family.

DENTAL - PPO Plan - no restrictions on choice of dentist.

Basic Plan – In-Network $1,200 / Out-of-Network $1,000
Provides 100% coverage for Class A (Preventive Care) with no deductible; 70% coverage after a $50 individual or $150 family deductible for Class B (Basic Restorative); 50% coverage after deductible for Class C (Major Restorative). All out of network services reimbursed at Preferred Dental Provider Fees.

Preferred Plan – In-Network $1,500 / Out-of-Network $1,000
Provides 100% coverage for Class A (Preventive Care) with no deductible; 80% coverage after a $25 individual or $75 family deductible for Class B (Basic Restorative); 60% coverage after deductible for Class C (Major Restorative). Class D - Orthodontia benefit (dependent adult & dependent children) at 50% coverage to $1,500 lifetime maximum (in network). All out of network services reimbursed at Preferred Dental Provider Fees

FLEXIBLE SPENDING ACCOUNTS - Employees can set aside money on a pre-tax basis to pay for certain out-of-pocket health and dependent care expenses. The Health Care account has a $4,000 calendar year maximum, and the Dependent Care account has a $5,000 calendar year maximum.

GROUP LIFE INSURANCE - the Medical Center provides this benefit at no cost to employees. The amount of coverage is dependent upon the employee’s job category. In addition, the employee can purchase additional life insurance:

Employee: 1, 2, 3, or 4 times salary, up to a combined max of $1,000,000 at group rates. (Amounts over $250,000 require Evidence of Insurability)
Spouse/Domestic Partner: $20,000 (Guarantee Issue) or $50,000 (Evidence of Insurability required)
Child Life: $10,000

SUPPLEMENTAL LIFE INSURANCE - Universal Life policies for self, spouse or dependent children are available at the employee’s expense after 3 months of service.

SHORT TERM DISABILITY - Employees working 30 hours or more per week are eligible to purchase short-term disability protection that provides up to 60% of salary to a maximum of $1,500 per week. Individuals must be disabled for a minimum of 14 consecutive calendar days and exhaust all of their Extended Illness Program (EIP) time before benefits may begin.

LONG TERM DISABILITY - The Medical Center provides long-term disability insurance for full time, Modified Work Week (MWW) and Week-End Only (WKO) employees. The Plan pays up to 60% of salary, up to a maximum of $12,500 per month, after a 90-day period of disability.

PAID TIME OFF BANK (PTO) - This is a flexible paid time off approach to be used for vacation, sick, holiday, and personal time occurrences. Employees begin accruing with first pay period. Employees accrue time weekly during the year based on years of service and job category. Maximum PTO accrual is equal to two times annual allotment.

EXTENDED ILLNESS PROGRAM (EIP) - This program provides income replacement at 100% for extended personal illnesses. Employees accrue time weekly beginning with their first pay period. Annual maximum allotment of 8 days; pro-rated for part-time staff based on hours paid. Maximum EIP accrual is 65 days. This program coordinates with short term and long term disability coverage.

RETIREMENT SAVINGS PLAN - Connecticut Children’s sponsors a 403(b) savings plan that is comprised of 3 components: Employee contributions (up to the IRS maximum annually), matching contributions and an annual core contribution. The matching contribution formula is 100% on the first 3% and 50% on the next 2% of pay. The annual core contribution is tiered based on the sum of an employee’s age and years of service. Employees must work 1,000 hours each calendar year to be eligible for the core contribution. All employees hired after January 1, 2010 are automatically enrolled in the Plan at a contribution rate of 3% of pay.

CHILD CARE - Day Care site located on the Institute Of Living, Hartford Campus.

EDUCATIONAL ASSISTANCE - The Medical Center encourages continuous learning and development. Education assistance up to $5,000 each academic year (pro-rated for part time employees) is available to employees working 20 or more hours per week after a six (6) month waiting period.
